Register EPO Patent Application in Hong Kong - Form P4

We have an EPO patent application that was recently published in Europe. We want take the first phase step of registering in Hong Kong. Specifically, we need completed the Patents Form P4 - "Request to record a designated patent application for a standard patent", in English. The EPO application is from a PCT application which is from a US Provisional Application. I can provide all the application numbers. The form needs a Hong Kong mailing address. The basic P4 fee is $380 HKD. So, I need the total fee in USD for you to both pay the filing fee and the legal fee to prepare and file the form.
