1:Lower Eyelid Surgery Introduction

• The eyes convey messages of emotion, vitality, and age. Over time, the eyes undergo changes that often make people feel as if they appear sad or tired.

• Each year, more than 200,000 people undergo eyelid surgery, or blepharoplasty, to correct these common problems.

• The aging process can cause your eyelids to sag and droop, which in combination with bags and puffiness can make you look tired and older than you really are.

• Eyelid surgery may help to freshen and revitalize your face by reducing the appearance of heavy and sagging eyelids, puffiness, and bags under the eyes

2:What Causes Eyelids to Change?

• Before you understand how eyelid surgery works, it is important to understand the underlying structure of the eyelids.

• Like the rest of the face, the eyelids are composed of multiple layers including

o skin,

o muscle

o fat.

• Over time, age and environmental factors, like sun damage, cause the skin's support network in the dermis to break down.

• As a result, the skin loses its structure and elasticity and becomes lax.

• The muscle layers also weaken. This weakness, along with the skin's loss of elasticity and the effects of gravity, cause the eyelids to droop and sag.

• Fat deposits normally give eyelids a full appearance.

• However, over time, fat is not only depleted, but is also shifts in response to weakened, sagging muscles, which results in puffiness and bags around the eyes.

• Eyelid surgery cannot correct crow's feet or dark circles under the eyes

3:How Does Lower Eyelid Surgery Work? (graphic)

• Bags and puffiness in the lower eyelid results from protruding orbital fat.

• Normally, the orbital septum holds the fat deposits in place. However, over time the orbital septum weakens, allowing the orbital fat to herniate forward, leading to the appearance of bags and puffiness.

• There are two common techniques to lower eyelid surgery

1. The transconjunctival (inside) approach works by removing and sculpting excess orbital fat through an incision in the conjunctiva. This approach is often used when there is minimal excess skin, or co-existing conditions such as a thyroid disorder.

2. The transcutaneous (outside) eyelid surgery to address all of the problem areas in the lower eyelid including the need to remove excess skin.

• Depending on the specific problems that need correction, the surgeon may treat the upper eyelids, lower eyelids, or both

4:Surgery Preparation

• An eyelid surgery procedure may last approximately one to three hours, depending on the extent of surgery.

• Prior to the start of your procedure, the upper eyelids will be cleansed and anesthesia will be administered.

• The procedure may be performed either with

1. Local anesthesia with sedation, in which the eyelids and surrounding areas are numb and you are in a relaxed state during the procedure

2. General anesthesia, in which you are asleep during the procedure.

• The surgeon will use a retractor to carefully pull the lower eyelid away from the eye

• This will expose the conjunctiva, which is a thin membrane that lines the inside of the eyelid.

• A small shield may be placed over your eye to protect it during the procedure.

• The surgeon will make a small incision in the conjunctiva with a scalpel, radiofrequency cutting instrument or a laser allong the conjunctiva, exposing the three orbital fat deposits.

• Before making the incisions, the surgeon may to place a suture in your lower eyelid to help keep your eyelid on stretch

• The surgeon will draw incisions lines beneath the lower eyelashes directly on the eyelids.

• This will ensure that the incisions follow the natural contours of the eyelid and the resulting incision lines will be as inconspicuous as possible.

• The surgeon will carefully make an incision in the crease of the upper eyelid utilizing a scalpel, radiofrequency cutting instrument or a laser.

• In order to sculpt the orbital fat, the surgeon mahy carefully clamp each fat deposit

• Then, the surgeon will carefully remove any excess herniated fat.

• This will reduce the appearance of bags and puffiness in the lower eyelid.

• Once that fat is cauterized, the fat deposit will be allowed to recede back through the opening.

• To access to the orbital fat, the surgeon makes a incision in through the lower eyelid skin an orbicularis muscle

• The surgeon then dissects the skin and muscle away from the underlying orbital septum.

• This allows the surgeon to pull the skin and muscle layers away from the orbital septum and subsequently expose the orbital fat in the lower eyelid

• This will reduce the appearance of bags and puffiness in the lower eyelid.

• Once that fat is cauterized, the fat deposit will be allowed to recede back through the opening.

7:Incision Closure

• 7i Internal Closure (their slide 7)

• After sculpting the fat deposits, the surgeon may close the incisions with absorbable sutures,

• Some surgeons may not use any sutures, and the incisions will be allowed to close on their own

• Sometimes the surgeon may place a suture in the lower eyelid and attach it to the eyebrow to keep the eyelid on stretch

7e –Skin Removal and Incision Closure – External (slide 8 and 9 from site)

• If there is excess skin in the lower eyelid, a small amount will likely be removed.

• The surgeon may stretch your lower eyelid,or ask you to look upwards (if you are awake) in order to assess how much skin can safely be removed.

• Once the amount of excess skin has been determined, the surgeon removes it with surgical scissors and/or cautery.

• After sculpting the fat deposits and removing excess skin, the physician closes the skin incision with sutures.

• Sometimes the surgeon may place a suture in the lower eyelid and attach it to the eyebrow to keep the eyelid on stretch

9:Lower Eyelid Surgery Recovery (old slide)

• As with any surgical procedure, you will likely experience some discomfort, swelling, and bruising which can be alleviated with cold compresses and pain medication.

• Non-absorbable sutures are typically removed in four to seven days, and you will begin to notice an improvement in the bruising around your eyes in seven to ten days.

• Although you will feel well enough to return to work in approximately a week, you should continue to avoid strenuous activity such as heavy lifting or exercise for at least two weeks following the procedure.

10:Lower Eyelid Surgery Results (new text)

• You will start to notice the results from your upper eyelid blepharoplasty within a few weeks as the bruising and swelling continue to fade.

• Internal approach

o One of the benefits of the transconjunctival approach is that there is no visible external scar

• External approach

o While you will have small inciunsions, they are usually well hidden by the contours of the eyelid and will fade over several months

• While the results of eyelid surgery are long-lasting, it is important to realize that aging continues.

• Ophthalmic Plastic and Reconstructive Surgery is a specialized area of ophthalmology that deals with the management of deformities and abnormalities of the eyelids, lacrimal (tear) system, the orbit (the bony cavity surrounding the eye), and the adjacent face.

• An ophthalmic plastic and reconstructive surgeon is an ophthalmologist (medical doctor and eye surgeon) who has completed additional training in plastic surgery as it relates to the eyes and their surrounding structures.

• Since such surgery can affects one's ability to see, ophthalmic plastic surgeons are best qualified to perform this delicate surgery and also provide any care that the eye itself may need.
