Changes to Mailchimp Newsletter layout

Hi There,
We have a very basic HTML template on Mail chimp and need a few basic changes.
1. Need a submit button which simply launches an email with some content for the To, CC, Subject and body texts that I will provide.
2. Also need a company logo added to the template. Just need a little logo to add to the template. Mono color -white. I will provide an ems file.
Should only be 30 mins of work
This work should be kept confidential as well provide our account details for mail chimp to the successful quote.
Also we have a number of other website changes we'd like to make so we will also need some further PHP mysql development done as well.
