form to email script

Add the ability to my php mail script to accept attachements and also html format

## Deliverables

I have this php script that is attached and I want to add te ability to have attachements.<br>

The headers in this script must stay as they are becase they will match fields on my form I will create in a program called Antenna web design Studio.<br>

In the message section you will see the form of a basic table, I still need to be able to write normal html in this section to be able to form my email template.<br>

Attachement feature must be able to accept a range of file types that I can easily define in the code, eg: doc, exl, pdf etc.<br>

In the headers it must have the ability to have a cc, bcc and reply option like my code does, The filed names in my sample code must be able to match what I use on my form that I design in Antenna Web Design Studio
