receipt formating for ready made paper -- 2

please check the attachment file

i need you to format my invoice to ready paper (maximum budget 30usd)

"printable paper form" is the ready print paper this already printer if you check the photo there is some note there i edit this i need to be print on that paper include the net amount

"invoice" - this is the current layout on my invoice i need you to change according to printable paper form

also you can check this code \application\views\sales\[login to view URL] to modified the design on the receipt

JUST NOTE THIS FORMAT IS A4 SIZE AND IF I ADD MORE ITEM IT SHOULD NOT CHANGE THE FORMAT FROM BELOW
