We are a SEO company and I'm starting to redesign sections of our website

The section I want re-designing can be seen from the attachment below

Important points:
**** Colours to be very similar/same as the existing banner I have
**** Must include a rocket AND the clouds at the bottom or somehow incorporate the bottom of the banner with the header where the logo is, so it looks seamless
**** I need a submission form designed for the following fields: Name, Telephone, Email, Website - TO BE INCLUDED WITHIN THE BANNER
**** The same headline to be used 'Skyrocket your Google rankings with Talk Sense today!'
**** The design needs to be striking, clean and professional.

I need the PSD file upon completion as will need to make mobile friendly/responsive, so please take this into account when designing.
