Simple PHP Web form with MySQL

The project was continuously delayed using various excuses. The project should be first completed by 22nd of May and was postponed to 30th of May (reasons were provided only after the delay), then postponed again to the 15th of June due to his health reason. We tried to show sympathy for him, hence accepted the delay till the 15th. However, despite that he continuously promised he would complete the project on time and provide daily status updates for us to review. The project is still less than 1/2 completed after 3weeks of work and required constant chase up on the project status (daily updates only lasted for a 3 or 4 days followed by little progress and response with excuses and empty promises). Do not think this guy is capable of following schedules and keeping promises, also seems incompetent at stages where simple query to extract data was provided by us. Very Disappointed!
