Design, Develop 1 static webpage with sign up form

The service provider delivered. But it is not something I could use. There were complaints of my requirements not being clear (which is partly true) and I had called this out myself on the project description. Made it clear that I do not know the design but only have a concept that i need some experienced pro to translate to design and a page....The freelancer was certainly enthusiastic and put in efforts...and i paid for it. I cannot use what was delivered and it was far away from my expectations.
