Implementing new subpage (on WordPress) on existing website

Hello,
I am looking for someone who could implement new subpage according to the project in the attachment.
New subpage should be implemented in two languages - polish and english.
I require to use wordpress because whole webiste was build on WP.
The new subpage is the application configurator, filling in the appropriate fields by the user results in the display of the appropriate price based on the algorithm. The algorithm is included in the attachment.
There should be possibility to easy change texts, images, prices and algorithm via CMS.
I will give PSD file and access to server where new subpage should be placed.
