Create a new family address list template

About 10 years ago I took over maintenance of a family address list for my mother's side of the family. I have continued to use the same MS Word format created by an aunt in the early 1990s as elder family members are accustomed to the format. However, inputting information is a bit of a hack job and I'd like to clean it up before I pass it on to a younger generation to maintain.
Requirements of the desired file:
1. Must be able to be printed to 1-2 standard 8.5" x 11" pages. This is a requirement as older generations in the family still prefer to print a copy and hang on their refrigerator or bulletin board. A "large print" version would also be preferred, but not required.
2. Must be maintained offline. No cloud-based service, whether free or paid, should be used. So no google sheets/docs, no [login to view URL], etc. Ideally this continues as an MS Word file, but I'm open to other suggestions.
3. Must closely mimic the original layout, organizing the names from oldest to youngest, keying off the eldest generation with their families organized below them. Example below:
Gen 1.A name
Gen 1.A Spouse
Gen 2.A (child 1 of Gen 1.A)
Gen 2.A Spouse
Gen 3.A
Gen 2.B (child 2 of Gen 1.A)
Gen 2.3 (child 3 of Gen 1.A)
Gen 1.B name
Gen 1.B Spouse
etc.
5. Must incorporate the following fields:
Full name
Mailing address(es)
Email address
Cell phone
Birthdate (and date of death if applicable)
Wedding anniversary
Existing format will be provided on inquiry. Current version contains personal info, so I'm not able to share to that attachment at this time.
