I am in need of a form to do the following:

allow users to make multiple selections of multiple products and have them total out so customer can decide what items they may favor over others.

For instance "joe" wants to rent a SHANTY for two days but he also needs TIPUPS and SPOON. He should be able to pick these items from a list of items on the page he is on (via link[almost like shopping cart]) and item will be added to calc. When an item is chosen a popup window will open asking quantity of days item will be rented {ie:joe wants SHANTY,TIPUPS and SPOON.} Popup will ask number of days for each item as it is selected. If item does not need a number of days selected popup will not happen(MAP is purchased not rented[multiple MAP will be handled as seperate item in calc]).

Item list should be editable by me in admin web page.

calc should appear like invoice at bottom of page.

calc should have dates requested from xx/xx/xxxx to xx/xx/xxxx

calc should have text box for customer to input location they want to reserve(this will be small-med size text box so customer can give us detailed directions).

calc should have google map so customer can pin point location of reserved area they want.

calc should have item delete for each item and quantity edit for each item.

once customer has decided what they want to order they submit request via submit buttom which sends email to me for review.
