We have two forms in Word format (attached) that we need converted to PDF format. As you can see these forms are poorly designed. We need the PDF version to be cleaned up (alignment, logo at top, etc.). Also, the PDF must use Form functionality so that the end user may easily enter data into text fields, check boxes etc. using Adobe Reader. Please send any questions you may have.
