Build a vue.js form for user input

Job Description:
This is a relatively simple idea that I would like developed in Vue.js
Essentially it would be a Job Posting form.
So upon clicking a "New Request" button, the app would start the process of creating a new request (similar to how this website allows a user to post a job)
There would be a bunch of questions, input forms and drop downs etc to help the user create the request.
Upon submission, the request will then be posted to a database via API. This step isn't required at this point though, one for phase 2
It's important that the form is very user friendly and visually appealing. I can provide the colour scheme and wire frame etc
The types of input on the form would include things like:
- Request Name
- Type
- Category
- Deadline
- Data Sources
- Budget range
- Description
- Licenses
- Attachment
- .....etc
These will be a combo of input, drop-down, attach, radio button etc
