Ok so here’s the story. We do group health insurance. Currently, insurance carriers require their own proprietary form for the health questions that people fill out. These forms require a lot of information that we don’t actually need and we don’t want the employee to have to complete every different carrier questionnaire to get quotes from that carrier.
The first attachment is a form that I’ve created. We use this to gather information that we need from each employee we do insurance for. I would like to build out a platform where the employee can answer all these questions and have it auto populate answers into the different forms for each carrier. We would also need a signature field through one of the larger signature verification companies like DocuSign that would populate across all the forms as well.
There are a couple of companies that do this but they are very expensive and include a lot more data collection and add-ons that we don’t actually need.
