Intranet web page program or macro or java C# VB code

I need to have up to 25 numbers passed to a query from an intranet web page or or macro or simple program which will return results of up to 25 lines of data. The web page should have a simple title like Delivery Route. With 25 blank input fields numbered 1-25 with the word start next to the first field the program will accept 25 numbers. I have already written the query to return one address. The program can use the same query over and over or if there is a better approach that is acceptable. The data returned is street addresses line by line. such as 123 main st city state zip. The address will need to be passed to mapquest or a similar service for smart routing. I already have the query written for one address retrieval. The query uses a UID to query a DB for an address. The intranet web page or program should allow the user to enter up to 25 UID numbers and have a request button to run and retrieve the result set. The result set should then feed to [login to view URL] or a similar web service. The option for Allow MapQuest to re-order stops on your route should be checked to allow the mapquest service to make the route more efficient. Make this a Round Trip should also be checked for the route submission. The program or macro should pass the data to the routing web site and submit the data with reordering and round trip options selected. The web page or data entry should execute those steps after the submit button is clicked in the webpage, macro, or program. Once that has successfully happened my users can use the mapquest web page to print directions. The database is SQL 2012 and the two OS which access this program or macro will be server 2012 R2 and Windows 7.
