Hi, The part that decides this build isn't the form, it's the status column. A flow that runs on "item modified" will fire again the moment the flow itself writes a status back, so approvers get the same email twice and the loop keeps going until you switch it off. It gets fixed with a trigger condition and a separate stage-tracking column, and that decision has to be made before a single branch is drawn.
