I need a an application for windows that will take two paper forms that i use in my business and allow them to be filled out on the computer. One will use simple check boxes with about 20. The other will be a form with blank spaces for typing.
When the forms are saved they need to generate a simple "canned" text document.
There needs to be a database because
each time my clients come in, we need to pull up their name and fill out the form pertaining to their needs.
