Hospital record verification is the top priority on every assignment—you will review admission notes, discharge summaries, billing sheets, prescriptions and confirm their authenticity directly with hospital administration. Alongside this, you will: • verify patient identity at the bedside or registered address, • cross-check overall claim authenticity, and • when the file involves a death claim, visit the local police station to secure the panchnama or post-mortem documentation. Expected for every case • A concise investigation report (DOC or PDF) that clearly states findings and recommendations. • Photographs or high-resolution scans of hospital records, patient IDs, and any police papers obtained. • Signed confirmations from hospital authorities and, when applicable, the investigating officer. All material must be uploaded to our secure portal the same day the visit is completed, with strict adherence to confidentiality protocols. You should already be familiar with insurance or hospital environments, medical terminology, and basic digital tools such as smartphone cameras, Google Drive, Excel or similar spreadsheets. Fluency in English plus the local language of the site is crucial for interviewing hospital staff. Assignments are issued on an ad-hoc basis.
