I have written some code to ask user 3 questions and take the input as one of the given options. Once all inputs are taken, chosen options are stored/appended in excel file in next row. Afterwords small piece of code records 60 seconds voice sample and stores it in the same folder with an ID. This ID represents the corresponding row in the excel file.
All you have to do is convert it into GUI using Tkinter (preferably) or any other framework that would work on Rapaberry Pi 4. So that options could be taken as touch input from the 3.5 ich LCD attached to Pi.
GUI will contain Forms on first window after NEXT is pressed, in the next window you will ask question 1 and give checkbox options to select from. Then after you press NEXT, next 2 questions appear. On the last question you can store the answers to three questions and the strings taken on the first page in an excell file using openpyxl.
On the 5th Window you will link a button to the recording function which is already implemented in my code. I will send you my code (works perfectly in command line) and complete details via private message.
