The Blind Hangers Shop

Hello Freelancers, my name is Vinny and I am looking for a fairly simple website site application that will collect client data like, Name, phone number, address, description of problem along with the option to upload pictures. Also i will like for the customer to have the ability to schedule an appointment online, but will require confirmation from admin. So basically the customer will choose the dates and time they are available and someone from the office will call them back to confirm. After the appointment is confirmed, on the admin/back office interface, I want to view the status of the client and I will like this to be view similar to how a restaurant cook receives his orders. I want the order to view as a ticket and I want to know how long the tickets have been in que or in line waiting to be serviced. There is a process in which service is carried out and I will like this to be clear in this application. For example: A client contact the company via website. On the website they will be prompted to identify or describe their issue with their blinds or window shades. The website will provide a drop down menu of common issues and pictures for the customer to choose and it will also have a section where the customer/client can give more detail description of what is going on with their blinds. When this information is submitted through the web application I will like for the information to go to my Email account or I need to be able to have access or interface for information directly on my server.
