Set up MySQL database & post collected data to partner's API

This project is to do the following:
1) Create MySQL database in Amazon Web Services account.
2) Data will be captured on the following site, using a form that will be similar to this. [login to view URL] (the new form is currently being created to capture the information below in item #3a - #3h)
3) The database should record submitted entries to the web form and hold the following data:
a. First name
b. Last name
c. Telephone number
d. email address
e. street address
f. city
g. state
h. zip code
i. IP address of computer submitting the information
j. Date / time stamp of when the information was submitted
4) Add form validation to only allow 9 digits for phone number field and 5 digits for the zip code. If an entry is less than the required amount of digits, do not let the user submit the form and provide an error message.
5) Add pre pop - Add mechanism so the link for the web form's page can be appended to pre fill consumer data into the web form fields. An example would be: [login to view URL]}&first=FIRSTNAMEHERE&last=LASTNAMEHERE&email=EMAILADDRESSHERE&phone=TELEPHONEHERE&address=ADDRESSHERE&city=CITYHERE&state=STATEHERE&zip=ZIPHERE
6) Post collected records to partner in real time, using the attached posting instructions. Record the post response in the database for each record that is attempted to be posted
7) For posts that receive a "success" response, fire a postback to the Everflow tracking platform. ([login to view URL]) The postback will look like this: [login to view URL]
8) Schedule nightly back up of the database for each night at 1AM PST
