oscommerce mod

I need an oscommerce mod that will place four required fields somewhere at checkout that ask for "Unit Name", "Realty Co./Owner Name", "Arrival Date", and "Departure Date". They must be required and passed on to the customer email and the admin email or order area. I don't ship anything, mod of the shipping to delivery and have these four fields would work. I need these required fields so i can deliver the rental items that i supply. This mod must show up on every checkout.
