help for a php script

Job Description:
I have a small ecommerce site (different sellers with several different products per seller). I would like to include at the bottom of each seller step and at the bottom of each product page this script (referal) script can be found here : [login to view URL] sorry this is a french website
This is a script where someone can put his name and email and the name and email of another person + a message to either recommend the seller or the product of this seller to his friends, relations.
I would like the form of this script to be put in a file that I can introduce in a wordpress page (seller's page) and with a function or other shortcode principle for each product of the seller. The program in the background will send a message for the seller or the product on the page where the site visitor fills out the form.
I need finished files in php mode and html form (or php code) to include in wordpress pages.
