Disqus API used for TripAdvisor Style Reviewing

I am looking for a developer to use the Disqus API to create some code that I can place onto my CMS template (currently CraftCMS but should be able to be placed on any web page) that allows the user/visitor to write a review in the same way one would add a review on TripAdvisor (see attached image). The additional fields required are "Review Title", star rating (out of 5), plus ALL usual Disqus fields and functionalities which can be found on the Disqus website and also demonstrated in the attached image. Important: The HTML should be customisable so that any front-end framework can be used (such as Bootstrap for the fields, FontAwesome for the star rating etc. Although this should not be unable to be changed at a later date in case another front-end framework such as Material gets used at a later date. Please do not just use the Disqus design.
