Create custom Word Press form

I am a website designer creating a website for a client [login to view URL] the website I am building is built using Word Press. Total off around 500 suburbs are allocated into 7 different sections which each of the 7 franchisees look after their relevant section. I will provide you with a spread sheet showing each of the 7 sections that allocate to each of the 7 Franchise the 500 suburbs making it easy to place into a database. What I will Supply you with 1. Login details to Hosting account Cpanel 2. Login details to Word press website Cpanel 3. The excel spread sheet with all 7 sections relevant to Franchise that looks after the area this includes the 500 Suburbs. 4. Fields for the form (less than 10 fields) How The Form Needs To Work My client has 7 franchisees. When the customer enters into the form the suburb where they want the job done the form will need to do the followinf: 1. A telephone number that is relevant to the correct Franchisee for the suburb entered will be displayed however it will only display half the number so the custom will need to click on the number to display the entire number. The reason behind this is so we can track how many people have requested the telephone number - in other words somewhere the owner of the website will need to be able to access and see how many times each number has been requested. 2. When the customer has entered the suburb they wish the work to be done in if they choose to fill in the field "Comments or additional notes" and hit send the email must go to the correct person who looks after that suburb area. 3. When the customer begins to fill out the suburb field the form will automatically say do you mean (and give suggestions of the suburb) i.e. if I was to want to type into the field the suburb Pymble and in the database it had 2 suburbs that started with P lets say Pymble and Puns and I started to enter P it would automatically say did you mean Pymble / Puns and If wanted I could click on the appropriate answer.
