Website based database system with .pdf output

Hi, we are looking for a website based system that will allow us to input information (paragraphs upto 100 words long) into a database, the front end user would then choose a series of options based on the database queries and can then print a .pdf document containing all the information that relates to the multiple choice choice questions in the front end application. i.e. we would need to upload a front end choice (drop down list) and add the relevant information to the database, so the client will see a range of options (questions) and depending on their choices the relevant paragraph from the database would be included in the .pdf output.
