Good day Freelancers: We require a simple web based form application for a Furniture Removal company that has a wordpress website that must do the following: 1. The user complete the form with their details: Name, Surname, Contact Number, Email Address, Origins Address and Postal code, Destination Address and postal code and will enter the quantity of each item on the list that needs to be relocate and submit. 2. The application will calculate the total volume by multiplying the quantity by the number next to it and. (see attached file) 3, The form with the calculated totals and user details will be delivered to the business owner and an email will be sent to the user stating that the form has been received and he will be contacted shortly. Thank you.
