See here for those fields: [url removed, login to view] Requirements: - coding must be clean and fully commented - all modifications documented in a readme file - ability to add extra fields in future preferably done in admin panel - ability to make certain fields required - coder must be willing to provide installation and after-sales tech support for first 2 weeks of script operation
