iPhone App - Mobile massage services

I am a mobile massage therapist.? I go to peoples homes to perform massage services (& limited spa therapies) for those that want it in the privacy & convenience of their home.? As the sole therapist doing this by myself. **Here is the scenario:** Launch the iPhone app...My company splash screen. I see a menu of services that I can check off what I want.? I pick a 60 minute massage plus a mini-facial then hit the RESERVE NOW! button. (Reservation is a $25 non-refundable fee but it is applied to the cost of services performed)? I enter my address info and email & cell and residence phone #'s and credit card info to make the reservation (I haven't decided if this will make my business less attractive. But I have my reason for doing this.? It is to prevent those that are not serious about it and I will have to have the materials ready & the time I need to make for preparations before I arrive at their residence.) The iPhone app will save this info so it is not entered every time but will ask if they, "want to use the previous info?" Give the option to change info if needed. It will need to meet Apple iPhone App standards (I will provide graphics) I will handle the submission process to the Apple App Store (never done it tho.) This app will be given out for FREE! ## Deliverables Looking for someone to create either a mobile site along with my regular site or perhaps more of an iPhone app for marketing purposes and positioning.? But I just want the best solution for now. The only updates to be made are the menu of services and maybe products in the far future.? I may want the iPhone app to have a "Go to website" button with maybe an "About us" page.? So maybe 3 pages total?? I have looked at MobileAppLoader (template based iPhone app's and see really low ratings - all it is is just pix of the place used as wallpaper and the rest is rather lame - see the Mobile Chiropractor) I also checked out AppWizard but they don't offer screenshots, nor have they answered my email regarding that. I currently use the standard business PayPal account that I am using for the "Reserve Now!" button on my website. Then it returns to my form mail contact page so that I know what services they want.? Since PayPal does not allow anyone to put "$0.00" for any service so, I wont know what they want from the beginning and only charge for the reservation fee via PayPal. Your insight and expertise on the best solution (simplest and cost effective) across the board will be considered and appreciated.? Perhaps it may be better to just do a mobile website since the cc form can be done there and be secure via PayPal? I am only providing services in the San Antonio, Texas area for now. The app name could be "mycompany - San Antonio" and the next one will be "same_companyname - Austin"
