Tell a Friend Script Integrated into 1automationwiz

Looking for a custom Tell-A-Friend script that can be placed on webpage after checkout. So after someone comes and buys a product through our site and checks out with our [login to view URL] shopping cart they will be redirected to a page with this tell a friend script on it. Tell-A-Friend should have the ability for the user to add up to five email addresses and first names. It should pre-populate a text field with message subject line. It should also pre-populate a text area with a default message that the user can then edit. Once submitted, the TAF script sends that message to the email addresses specified AND sends the site owner an email letting him know the names the TAF just sent the message to.
