OSCommerce shop needs new pages - repost

Hi,
My webshop is osCommerce Online Merchant v2.3.1.
I need a good programmer to create new things on the webshop.
1) The webshop does have a real store. In this store we need to be able to create a new order ID, with the customer contact details (not required), the product name and also the amount and total amount. We need a new .php for this, and it has to be looking good, because this screen will be used in our shop. I have made a screenshot how this new page has to look like. If somebody can make something that the amounts are going to be calculated to a total amount, this would be great! The prices are in Euro's, €. We must be able to print this form in a proper way.
2) At /admin/[url removed, login to view] we want Insert new statuses. I want a message behind every status. As example: changing the status "Shipped", I want to send a standard message with info what this status means. We need 5 of these statuses.
3) Above each category we need a banner, to explain / tell something about the products in these categories.
