Looking for a back end form to be created for my users / employees to access. They need to be able to input data from paperwork I provide them. They would need to have a log in / password to log in to the back end, in which they are presented with a page to "Add Record" or "Search Record". The Add record would be used to input data which I specify. The Form will be about 20-30 questions, and may be multiple pages. Once form is submitted I need confirmation page.
The SEARCH page should be able to be organized according to the fields in the form. For example, I may want to SORT by age, sex, or history, last contact date, etc. I would specify.
