changes in php form and SMS sending

Hello bidders, I need some urgent changes to be done in my existing site. here are those. I am attaching snapshot of how the final page will look like. 1. Please add "add client" functionality on "add order" form. so the user can create client without leaving the page. disable other fields till this step is completed. 2. Please add new fields for "cab booking" like from-to, trip time etc. Please add amount field against each service category which will be passed in invoice instead of rate mentioned in current table. Please add driver selection in the form using a pop up. 2. When the form is submitted please send SMS to new client. we will share you the API for this.
