Wordpress Theme needs customization

Recently a developer that I was working with had to leave town. I have an existing Wordpress template that is almost finished. I have three pages that need to be modified so that a user can input information under a "custom options box" within a posts page so that the content will then show up under the corresponding page that will be driven by categories.
I have attached a mockup of what the current pages look like and what I need. Let me know if you can complete my requests. I do need a fast turn around time.
