This is a book request form, similar to a very simple shopping cart. The user will fill out some basic information about themselves, name, address, etc. Then they will select up to four books that they wish to receive complimentary copies of. This request form will then need to be submitted via email.
First section will be the user information form of about 15 fields, including text, check-box, drop-down select and textbox. I can give you specifics of what fields they'll be. Assume basic contact info and some simple survey information.
I have a MySql Database with the book information. There are two tables - one has 16 book categories and numbers assigned to each. The second has the number category from the first table, the internal book code and the title of the book. This info corresponds to accomodate a dynamic drop-down list and an adapted script in PHP. A user selects a category of book, then a second drop-down displays the narrowed book list. THIS CODE IS ATTACHED. This can be changed if you've got a better way of doing this.
I'd like it if after selecting each book, the book selection(s) are displayed at the top or bottom of the page as they select more. I'm guessing that each selection will be added to a session array and then the page is refreshed. Since only FOUR books can be selected, the user should also have the option of removing/deleting each selection.
After the user has selected all their books, they should go to a summary page which shows all their form information (contact info, etc) and the books they've chosen with the option to make adjustments either on this page or by sending them back to previous pages. Upon SUBMIT the info will be sent to a specific email address.
