image upload script instalation

hi i have a simple one page website i need a formwith image upload this form should allow customer to locate a file image on there computer and upload it then let them choose a size and then let them choose from 4 colouring options then a comments or notes bit they can type in to then a buy button this button will then send them to paypal were they will pay for the item so in effect it will go like this upload image --- choose size from 4 or 5 different size options ----- choose sepia,b/w, full colour or custom ----- then a notes section --- then payment ordering bit.... then i need this to fit in to my page and have a few nice buttons to work it and this must also if possible work from a html page need this done as soon as possible please.
