Custom Survey Development

I am in need of a php developer who can complete a project recently started. The project is for a custom survey development tool. All the specs and requirements can be found in the attached doc. Basically, this project is for creating the survey creation wizard, finishing the survey listing functions, and creating the edit functions. UI (user interface design) is being worked on separately.
