Modifications to existing Joomla PHP Function

Hi. We have a form that is using AJAX and PHP that needs to have two simple drop downs added to the form. The drop downs are simply a quantity for additional people using this form.

Also, we need to have the ability to display information from a simple select statement into a page from existing mySQL tables.
