Need assistance with project like ratemyprofessor

The project is entirely similar to ratemyprofessor, but was created for a local school district. I used an open source base for the project, but other than that have completed development on my own; however, I need assistance with completion..

Basically, what I need completed is the 5 category rating function; I have already developed it for the most part, but the php and tpl files need edited to reflect the change across all pages.

Examples below;

on teacherprofiles, only the overall rating is functioning.

on the 'review' image you can see I have added a couple categories..with working js and an updated tpl file; but only overall rating is registering within the database..this is another page that will need to be finished

lastly, on [url removed, login to view] you'll see the users profile; it needs to be updated to reflect their ratings on all 5 categories..and possibly show the teachers overall rating only or their ratings overall for all 5 categories...

This is a simple project, so the budget is not very high; however, I need the modifications completed rather quickly..so keep that in mind. I am not looking to pay an outrageous price for simple script modifications across just three pages.
