PHP Questionnaire with database

We have a website, which will include a page for people to apply for financial aid. We want to create a Form which includes around 100 questions, ranging from multiple choice to simple fill the answer, attachments, and pictures also will be added to the form. This form will be as a wizards, having 4-5 maybe 10 steps, after completing each page, the user will be directed to the other page. However, there will be list of requirements for some of the fields. For example if the user chooses country different than France, the user wont be able to continue to the next step and he will receive a message, if his age is less than 18, same thing. We will give you the questionnaire in Word/PDF form. You task is to develop the front end, wizard style form filling, we will also give you a list of requirements, where the process will stop if the user does not meet them with the appropriate message. The user wont be able to continue unless, he changes the entry. There will also be some mandatory and optional fields. After a user completed a form, he will get a confirmation page and an email. We will also receive an email saying that someone has completed a form. We will give you the content of both. A special CMS page will be created for us to enter and see who has submitted there forms. A nice, user-friendly, profile will be created for every submitted enrty. We will be able to enter, view, edit, the profiles. Also another important thing, we wont to be able to print the profile and it should look similar to the Word/PDF document of the Original Form, yet with the entries of the profile. The Task is required to complete not more than 10 days. With full support along the progress of the project and support after the finalization for modification (of course, we will discuss an appropriate payment for each modification after the completion.)
