We have just aquired a web site from one of clients.

pakmailautotransport(dot)com

So I moved it from the old server to out new helm server and now it seem that there is a file missing.

I need to resolve thie Emergency style!

I need either this form working as quickly as possible or I need a new php form instead if it takes the same amount of time since we will be converting this site soon anyway.

Please help!!
