7328 Formmail script

I need a formmail script. Here are the fields I need. first name last name email address Phone number work home and one other phone number field for foreign phone numbers Phone number must have 3 fields area code which allows just 3 digits and no less phone prefix which allows just 3 digits and no less rest of phone which allows just 4 digits no less A pop up should come up with instructions if someone trys to enter less or more than the required number of digits and only digits should be allowed. Then there should be another phone field for foreign phone numbers which should allow up to 30 characters or numbers for foreign phone numbers.(Non US or Canada) The form should require that all fields be filled out. Except it should require that only one phone field be filled out. Also the email address form should validate that it is indeed an email address not just any digits or letters. The form should redirect the user to the url of my choice and should save the lead information to a file on my server and it should also email me the information to the email of my choice. The ip address of the person who fills out the form should be recorded and sent to the file and the my email address of choice.
