Remote Testing Tool

A WEB enabled data driven mobile app with dynamic form creator and storage, where as a desktop app can create a form and make it available to all app users. App consists of a multiple Question and answer forms in a multiple choice format (1 right answer out of 5) implemented as toggle push buttons on the canvas. The results a stored in a DB under a unique user profile id, so that they can be later analyzed remotely with an analytical tool such as Office Excel. A separate User Profile generator form would be necessary as well, holding standard User Demographic data. App launched through LogIN or Creat an Account button. Capability to LogIn and LogOut. Change Password, Lost Password. Account verification via Email and/or SMS with automatic code reception and plug-in.
