Requirements can be met by creating a SharePoint site with forms where health workers enter their shift details which will be available for other workers as well and they will be able to see all recorded notes. However, if your Office 365 license has Microsoft Teams included then a Teams channel can be created and all health workers collaborate very easily and enter their shift details either in a form or Planner app.
