Create a Mac OS app to help mount network shares -- 2

I'm looking to create a MacOS app that will help mount 2 static network drives shares as well as multiple network shares based on the contents of a text file located in a public network share. We have a Windows vbs script that does this and I’m looking to create something for our Apple users to do the same.

The interface is a simple Username and password with ‘Connect’ and ‘Disconnect’ buttons as well as a 3rd button that directs users to a help request form website. There is a third field that is just a drop down list of 2 different domain sites that needs to be appended to the username in order to connect. When I Command-K and manually mount a network drive, MacOS asks for my username/password and when I connect to the share my user name would be Domain\Username.

When the user enters their username and password, our Windows version goes out to a network folder and finds a matching text file named after the username that contains network share locations that each specific user has access to and then proceeds to mounts them.
