I need a survey created on a Joomla 1.5 website.
The survey should look (design) exactly like this one:

The style/look of the survey should match the one on that link, and it should also operate like that one.
The questions should be:
1. Do you find it hard to concentrate?
2. Do you have days where you have no confidence?
3. Do you feel tired a lot of the time?
4. Do you find it hard to make decisions?
5. Do you find yourself taking your worries out on those around you?
6. Do you often find yourself getting angry or irritable?
7. Do you find it hard to say no and end up with too much to do?
8. Do you feel tense and 'knotty' in your back and shoulders?
9. Do you have problems sleeping - either getting to sleep, waking up in the night or suffering from nightmares?
10. Have you been suffering regularly from headaches or an upset stomach?
11. Do you find it difficult to clear your head at the end of the day?
12. Do you find yourself comfort eating or drinking? Or have you gone the other way and lost your appetite?
13. Do you constantly feel like you are up against the clock?
14. Do you have mood swings?
15. Do you find yourself snapping at your friends, family, colleagues or partner?
Stress Test grading. Each answer you say YES to is classified as 1 point.
Tally score to reveal the total number of YES or NO responses. eg: 10 out of 15
YES answers 1-5 = Low
YES answers 5-10 = Medium
YES answers 10-15 = High
