Toy Chrome extension to modify search results page

I need a working example of a Chrome Extension that can modify the presentation of my Google search results.
I'm looking for very readable sample-quality code that:
- Must run in current Chrome versions (e.g. 45). Does not need to work in any other browsers.
- Can be installed as a Chrome Extension.
- MUST NOT depend on TamperMonkey or any other 3rd-party tools.
This Chrome extension should:
- Modify the results of all Google searches (at least on [url removed, login to view] and www.google.co.il. Other countries are optional).
- On each search results page, modify each search result, adding the phrase "hello, I see: <url>" in green, with the result's URL substituted in place of "<url>"
