Custom survey question scripting in Alchemer

Job Description:
I need to create a better look and feel for a horizontal radio button question in my survey platform - Alchemer.
I want to be able to display a 0 - 10 point scale horizontally in a desktop environment and potentially have it display vertically if resized to mobile or tablet.
I would like the 0 -10 point scale to have headers explaining the range that they represent (i.e., 0 - 2, 3-5, 6-8, 9-10). Attached is an image that could represent how the question should display.
