Chrome extension for Filling the form

I want to create a chrome extension where I want to fill up the details of a form.

Pages

Userid & password for Signin to my website [No Registration needed, it will be done on website]

When I open a form page, It should read the fields of page like Name, Email id, Mobile no and fill them from my website, as I have signed in from chrome.

My backend website is Nodejs, Mongodb...using passport Authentication.

Note:

Form fields will be specific only...not open ended. I shall give the names of the forms to look out for.
