I need a feedback form that i can put on many of the websites i manage. I want to host the form so i can be sure the mail server etc...work and that the form get submitted correctly.
On the client server i want to have some js on thier page that is looking for a div. The div on the page will be something like div class=feedback
the js will put a image here that is clickable.
On click of the div area/image we will load a new window with my form inside of it. I think this will have to be an iframe since my form is on my server
Next the user will fill the from ( will show you an example of what i want) they will submit. id like to make some animation here like ajax that lets them know it is sending. they will get a copy of what is sent via email and i will get a copy of what is sent via email
We will use flat txt for the email styled but not with html just good use of notepad
I also want to know the page it was sent from. i think you can insert this in the form via the http request
I would like the form to have several pages, the user will click next and then the page will slide to the right and then the next question will slide in from the right. Jquery is good here.

but each question will be on a different page that slides in and out using the js script.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
